What they could improve on is doing their job and taking care of the residents that are paying their wages

Understaffed and the staff that is there isn't trained for adequate care of residents. Staff is outside smoking more than inside caring for residents. Facility isn't kept clean. Poor management. We had to hire someone to come in and provide personal care due to lack of care by staff.

The day my brother moved in the air conditioning was not working. It was set at 72 but it was 81. Two months later, the air is still not working properly. We were cleaning his place in a 84 degree room sweating (the thermostat was set at 70). We have said something to them several times and they keep telling us they will send someone to fix it. If they had to sit in that room day in and day out, I am sure it would be fixed by now. I don't believe he is getting the assistance they told us he was going to get when he moved in. There are some people that are nice to talk to but unfortunately the facility falls short on providing the assistance they claim to do.

We were very dissatisfied with Cedar Grove they use to be a very good place to stay they will tell you that they will do all kinds of stuff to help your family member and talk very nice to you but the care that they get is very minimal you pay alot of money to be taken care of they just try to fill a room but don't take care of the residents like they are supposed to and beware there is a cleaning fee not cheep upfront and then when you take your family member out and you will if you care about them they will charge you another month so good luck we have had better luck in Ohio

My husband did not belong there since his memory was not nearly bad enough at that time. However they just wanted to fill a space so it didn't matter to them. My husband is now in assisted living and doing very well. So much happier.

I was disappointed to discover that the facility wasn't truly handicapped accessible until after move in. We had to take doors off of bedrooms to fit wheelchairs through. They came to rehab and personally assessed my parents and said that they could take care of my dad, but then they were not set up for his needs. We ended up doing most of his care. We were working on moving them when my father died.

The staff who worked were caring/ competent, but they were so understaffed it was ridiculous. Patients with dementia are wandering around looking for staff and none are to be found. One day we went to visit, and walked over the whole facility looking for a staff member to help someone in need, and finally found the only 2 aids on duty out smoking on a side porch where they thought no one would see them. Also horrible urine smell throughout. Also price gouging/ trying to bill for months after our family member left there. Also say they can handle nursing patients when they can't/ should stick to accepting only assisted living level patients. Also let our family member get horrible bed sores by not having enough staff to do adequate diaper changing routine.

They were kind, but not trained to turn patients or clean them. Not aware of need for cleanliness and preventative care.nor concerned for mental well being. Didn't get her out of bed. She could stand and pivot but needed assistance. They Did not do this. They are not trained for professional aid. I was an I U nurse so I know what should be done as an aid. Also didn't warm up her food/didn't help her set up tray/didn't t make sure she liked what she had( I had bought her some things that could be warmed up) Only took a few minutes. They also promised me they would be able to care for her. She really was easy and really sweet lady. Makes me sad to think about. I almost reported them a few times but I did not. Hoping they get their act together

If I could give 0 stars, I would. At first, everything seemed like peaches and cream. Then a certain staff member was hired. She treats the residents horribly and when we reported her, management was "shocked" to hear it (even though multiple complaints have been reported). I realize they are short staffed and help is hard to come by, but at the risk of mistreating the residents, shame on Evergreen. Please think twice before sending your loved ones here.

Unfortunately, they dont providing good service. CNA's do the majority of interaction with residents, they are not allowed to "assist" residents with very basic needs. The staff rarely answers the phone if you need to speak to them. I've only seen them enter resident's unit, hand them their meds for the resident to administar themselves. They stay maybe 3-5 minutes. Most leave immediately and my loved ones medication dispenser doesn't work and they dont return to assist. Therefore my loved one goes without the med. This is a real issue that keeps happening. The food is not suitable for ANY of the residents-often times hot dogs and potato chips to these residents will all kinds of health issues. And the laundry facilities are often broke down. Clothes dont even smell fresh. There are mouse traps in the halls on the upper floors!! Even post renovation. There is NO transportation provided ie, outings or trips to stores etc.

My mom was here 2 weeks before being sent to the hospital for 2 bad infections. They advised me to stay away as she was upset for being moved there. All calls to check on her were so positive and how wonderful she was doing. This is not what I saw upon my first visit. She was out of it and shaking. Next day they sent her to the hospital where she almost passed twice during the night. She is now immobile and suffering other conditions because of the infections. Also 2 checks were lost. How can you expect someone to trust the care of a human being when they lose checks?

A funny thing happened on move in day–another group was moving their loved one out. A man with this group sourly stated to us as they exited the place, “good luck!” Deductive reasoning indicated this family may not have had a happy time with this facility. This review is being written in fall of 2023 not even a full year after. The issues: *High turnover of office staff and leadership *Constant billing issues – The monthly assessments were all over the place and inconsistent. *The person who on-boarded our family indicated that HW will coordinate travel to doctor's appointments. This turned out to be incomplete information. Staff shared that diabetes health management is provided. This of course also turned out to be non factual. *During Facetime calls with my relative, assigned staff will come into the apartment and immediately start yelling. *The seniors who aren’t quite great at toileting are allowed to fester in their neglect. Just walk the halls and you will experience this.

Bad to very bad: Food (Skimpy and sometimes not edible) Wait Staff (Rudest ever encountered) Many CNA's (A few nice ones) Diet (No special diets there, high fat and sugar) Therapy (Advertised but never saw) Bed Bugs (Always infested) Felons (They allow criminals grand theft auto and stealing from other residents and staff)

This place has awful management ever since they transitioned corporate companies. It seems like no one wants to do their job and their priorities are completely out of order. The management team needs to do a way better job especially to call themselves a “care facility”

Mom’s stay at Traditions at West Park Place turned into a horror story, judge for yourself. I have months of horror stories but due to character count I had to delete a lot of this review, My visit [removed], Construction dust from installing a sprinkler system wasn't cleaned up until I complained. Mom's furniture was covered in this. She had cellulitis on her leg and had never had skin issues before. [removed], Inhouse nursing care [removed] OVERLOOKED MY MOM FOR 5 WEEKS. No baths or meds. My visit [date removed], Moved Mom to assisted living, worst decision ever. [date removed], Mom was taken to St. Anne's Hospital for severe diarrhea. She was dehydrated. As it turns out the aides were helping Mom fill out her meal tickets and adding beef and pork to change up her meals so they weren't so boring, their words not mine. Mom hasn't eaten beef or pork for 40+ years. The change in her diet at 95 gave her severe bowel issues.

We picked this community, because the food was excellent. Also, the community was wise. I really liked the financial staff and cooks. Originally it was highly recommended. It went downhill during covid. It was unfortunate. We found them living in filth. The were not cleaning their room.

Make sure they have cell phone and or alarm the office staff real good until she passed then they refused to let me in to find the car title I wouldn't recommend that place I hate the way I feel about it but mom was liking it.

Horrible experience. The smell in the building was horrid. They allowed the lady across the hall from my family member to save/hoard her own pee in jugs all over her room. The smell was so bad, they put towels under the door to try to block the smell, but it didn't work. Staff constantly complained about how their backs hurt in front of my mother in law when they had to help in and out of a wheel chair to go to the bathroom because she weighs about 250 lbs, and made her feel like she was a nuisance when she needed the bathroom or anything that required them to lift her. Horrible quality of care. She rarely got bathed. They dropped her so many times, she was bruised from head to toe. The hospital questioned us on her care because of all the bruising from being dropped on a regular basis. the room was so small they couldn't hardly get a wheel chair beside her bed. Just a horrible place. And very expensive for poor care.

Worse place Ever! My mother was a resident there. Her room and her appearance were horrible! They let the carpet in her room get filthy! They let her were tattered and stained clothing. When she began losing the use of her legs, they NEVER attempted to exercise them. As a result they became discolored. She had several personal items stolen. The nursing director, Storm, and the facility director, [name removed], NEVER apologized for ANY of it. They just acted as if it was no big deal. They would give half-baked explanations for everything, and then refused to speak my sister and I about things. What is worse, the state agency responsible for licensing them and the management company that owns it, did not do much to help. They "supposedly'' had inspections and found nothing wrong. The State representative basically stated she did not want to get involved. Later two aides let my mother fall. She had to go to the hospital due to this.

I would tell others to make sure that you have an assessment done before moving a loved one in to make sure it is a good fit. We found it was not a good fit for us and I believe they were trying to just fill the bed rather than seeing if my husband was truly a good match.
