I moved my wife in Autumn Leaves 8 months ago. Since that time I have seen my wife decline and need more and more assistance. Before COVID I visited my wife every day and we had lunch together, anytime I asked a nurse for assistance someone was there to help. The staff is very attentive and eager to please. The facility is clean and always smells fresh. I would recommend Autumn Leaves in Tulsa to anyone needing to move a love one into a memory care.

I love Autumn Leaves Tulsa and the care they give my AUNT [name removed]! From the minute she was placed there it has been comforting. She has been treated kind and with respect. The workers have done a great job and the facility is clean, calm and always having some kind of activity with the patients. Its a safe place for the people who need help. I can't say enough good about Tulsa Autumn Leaves staff. Way to go! The [name removed] family!

My Dad and his wife have lived in the facility for 3 months now. The staff has taken great care of my father on a day to day basis, and when special assistance is needed. The staff are always very friendly and accommodating. If the staff identify a personal issue with either my Dad or his wife, they share the information with the spouse so they can work together to resolve the issue. The cafeteria staff are friendly and consientious of their meal needs. Both residents have special needs for their meals. The staff work to provide options that will allow them to have a healthy diet while not feeling like they are missing out on the foods they really enjoy. One other aspect that I've truly appreciated is that the facility works to meet the needs for my dad, which are part of the monthly rent, not an additional ale carte charge. One last comment, the staff really want to make sure the residents are engaged. They actively include them in events that are scheduled, make sure they are aware. They are also open to new activity suggestions. My dad and his wife have really acclimated well to this new environment and have become active members of the community. Autumn leaves was a great environment for a loved one every facility has turnover and keep in mind these are Alzheimer's and dementia patients some things get broken and every resident has the right to refuse care... I'm not taking up for the facility but you do have to look at things from a bigger picture and really things that need to be dry cleaned like real leather shouldn't be there but to call caregivers stupid is uncalled for.... I as a past family member think some things can be better but the love and caring you cAnt get in a lot of places keep up the hard work ladies and gentlemen....

Autumn Leaves is great community if you are looking for memory care for a loved one. My father has been here for almost a year now. The office staff is friendly and will address any concerns you have. There is some turnover in caregivers as to be expected in this industry but the important thing is that they bring in Agency staffing so that they are not short handed. Top Notch community when looking for memory care!

The staff is great, and overall, my mom has received good care. Her apartment is the perfect size and meets her needs well. The staff members are kind, helpful, and genuinely seem to care about the residents. The biggest area of concern has been the food. Unfortunately, the meals have been disappointing, and my mom often does not eat the food that is served. Overall, I would still recommend the community and I am hopeful that the food situation will improve. I am not sure what steps are being taken to address the concerns about the meals, and I will be interested to see whether there is meaningful improvement going forward.

How about this: It's a small, cozy facility, which is what I was looking for. My mom likes the food, although the quality could be better on weekends. The staff is friendly, but they can be understaffed, especially on weekends. The facility is kept very clean and cheerful.

The building is relatively new and in a good location. The staff is friendly and fairly prompt. The seem to be caring of the residents although I've never seen any recreation other than tv rooms and a nice library that is used mostly for group visits. The place is very clean and virtually odor free. The food could use a little help even though the regimented schedule has mom putting on weight. Seems a bit pricey but is about half the price of round the clock home care.

Our experience at chapters has been positive. My mom is very happy there. They've had turnover with people and ownership, but I think it's all been pretty good. As long as my mom is happy there, I am happy.!!

After touring a couple of facilities, we decided on another facility. We really liked [name removed], but felt the other facility had more medical staff on site and available. This was our main determining factor for our family member.

Chapters is a great find, truly. The staff has been very good to deal with, very personable and responsive. There is a nice culture there, you can tell. Residents are in good shape and kept engaged with activities. Mom isn't super aware most of the time, but I know from talking to other residents that there are folks there engaging her and looking after her. She tells me that the food is really good. As she is a former Missouri State Division of Aging inspector she had me pull their inspection reports and review them prior to her moving there and I was happy to find that in the 7 years accessible to the public there were only a handful of noted violations, and those occurred in the 2018-20 timeframe under different management with nothing "customer facing" or directly impacting resident quality of life or safety since that time.

The facility is very clean. The staff provided great care for my dad. The ladies at the front desk were friendly and always greeted my dad and I with smiles and great conversations. The administrative staff was friendly when I went in to tour and sign lease paperwork, but became standoffish once my dad moved in. My dad did not like the food but overall, the facility is a great place to live.

They are kind and positive. I would like to see my mother participating in more activities. I think she misses out on activities. My mother's experience has been good.

Chapters Living of Tulsa is a smaller facility than most, only 40+ residents. The present admin & mkt person are working to improve the location and esthetics with repairs, painting, new furniture, etc. since taking over the building a relatively short time ago. The floor staff is great and caring. They are actively looking for an activities director. The building is clean and no hint of "odors".

Well, this is my first experience, nothing to compare. I have been impressed so far. All the ladies are very attentive to the patients. Usually very clean. She is in a shared room and has plenty of space. I don't know about activities.

I think the staff is wonderful very friendly. The food could be warmer although people who have been here longer than me say it is much improved. I enjoy many of the activities but might be to early to judge. Their staff is definitely their strong suit, they really care about their residents from top boss to housekeeping and kitchen help everyone is focused on the residents experience

There were good discounts offered. They had the services I needed available, some at an extra cost. The facility was tidy and adequate, but not fully renovated. The one-bedroom apartment was smaller than expected, only a little over 300 square feet. This was the first place I have toured; therefore, at this time, I don't have others to compare it with.

Addington Place is now Chapters Senior Living. My sister is very happy there although she is not very thrilled with the food . She enjoys most of the activities. She has never been one to do crafts so she only does a few of the craft activities but she likes the games and physical activities and she enjoys the other residents.

The location, amenities and friendly staff. The community is new and very clean. I would definitely recommend Kingsland Walk to others. I was just there today visiting my father
